Today, due to the rapid growth and development of technology, organizations can remain competitive if they can design all their activities based on creativity and innovation. Universities are not exempt from this issue and they must be able to draw their performance based on creativity and innovation in order to compete with other universities. Therefore, the purpose of this research is to provide an evaluation model of organizational performance with the approach of creativity and innovation in universities. The research method is qualitative and the type of Grounded Theory. The participants of the research were university faculty members (with at least 5 years of service experience and at least a doctorate degree), and the sample size after theoretical saturation was 12 people and in a purposeful judgment. selected. Data collection was done through semi-structured interview and data analysis using thematic method (open coding, axial coding and selective coding) and using MAXQDA software. And with the method of confirmatory factor analysis and structural equations, the validity and reliability of the data and variables and the conceptual model have been investigated.
